It's a time consuming, yet of course, a very rewarding thing working to save pets here in our county.  We are all regular people with jobs, family, homes and other responsibilities, along with the responsibilities of rescues.  I hope that having a few of the documents online, that are needed to keep things running smoothly and in order will make things a bit easier for everyone.

In order to print these documents, click on the link to go to it, then in your browswer, click file, print. You can now manually fill in the document and mail it or hand deliver it to who ever needs it.

Membership Application - Membership application printable for you to pass on to potential new members.

Rescue Information Form - This form is a requirement to be completed on EVERY animal taken in by CHS.

Owner Surrender Form - a form that needs to be completed by the owner of a pet who wishes to relinquish responsibility of pet to CHS.
